Expanding Rural Access to Integrated Community Health Services
The goal was to create a multi-service care environment that supports social determinants of health by co-locating outpatient clinics and shared resources within a modernized facility.
The Rural Health Village is a transformative community health initiative developed with Memorial Hospital to address needs identified through the Community Health Needs Assessment. Phase 1 includes renovating 21,500 SF of the existing building to create modern outpatient clinics and designing new additions to strengthen the facility’s identity and improve circulation. By co-locating multiple clinical service lines that address social determinants of health, the Rural Health Village expands access to care and promotes a healthier, more connected rural community. It is anticipated that design and construction will be completed in multiple phases to meet community needs, manage construction costs and make the best use of the existing facility. Phase 1 spaces include family practice and primary care clinics, diagnostic imaging, lab draw and testing, administrative and common spaces and food service and dining.
